Case Management
Members who are facing a complex health problem or a chronic condition can enroll in Case Management. Florida Blue has condition programs that can help members stay on track and lets members choose how they want to engage with us:
Nurses and other care professionals from our Dedicated Care Team will work hand-in-hand with members and their physician’s to keep members on the path to achieve their health goals. Between consultations with a member’s physician and care team, the nurse case manager will track their progress and stay in touch via phone and email.
Case management provides help to members managing ongoing conditions with personalized assistance for conditions such as:
- Diabetes
- Asthma
- Chronic obstructive pulmonary disease
- Coronary artery disease
- Heart failure
Dedicated care nurses also help members with rare conditions by educating them about their disease, emphasizing the importance of taking medications as prescribed, coordinating with high-cost specialty pharmacies and education members on other resources that can help with high member cost-shares that can prevent medicine compliance. Rare conditions covered include:
- Crohn’s Disease
- Seizure Disorders
- Cystic Fibrosis
- Multiple Sclerosis
- Parkinson’s Disease
- Sickle Cell Disease
- Rheumatoid Arthritis
Case Management also covers serious and complex cases, like:
- Organ transplant
- A voluntary full-service program that provides access and prior approval facilitation, education and information during all stages of transplant, proactive management based on needs post-transplant and collaboration with provider and clinical team.
- Cancer treatment
- We assist members who have been diagnosed with cancer regardless of the type or stage or proposed plan of treatment. We provide education and emotional support, aid with symptom management, and provide support with medications.
- High-risk pregnancy
- A voluntary full-service program that provides access and prior approval facilitation, education and information during all stages of transplant, proactive management based on needs post-transplant and collaboration with provider and clinical team.
- Neonatal intensive care
- This program provides pediatric case management for premature babies when they are admitted into the NICU. We provide access to information, education and services with have a positive influence on the health of the infant an improve clinical outcomes as well as collaborate with the provider and clinical team.
