Chronic Disease Management Programs
UFHCFs disease management program provides several resources for participates to help support them in their pursuit of a healthier lifestyle and better self-management of their health concerns, medications and chronic health conditions.
UFHCFs DMP is completely voluntary. The intent of the UFHCFs DMP is to provide resources for participants who are currently diagnosed with chronic conditions and /or who are at rising risk of chronic conditions.Â
