Plan for UF College of Veterinary Medicine Residents and Interns
The Options plan offers you three network tier options. Tier 1 benefits, provided by the GatorCare Network, offer the best value, with a lower deductible and low out-of-pocket and co-insurance amounts. Tier 1 includes hospitals, physicians and providers in Gainesville and Jacksonville. Tier 2 benefits apply when you receive services from Florida Blue’s NetworkBlue providers, which typically result in higher out-of-pocket costs. When using Tier 1 and Tier 2 providers for wellness and preventive care, annual physicals and labs are offered with no out-of-pocket expense to you. Tier 3 benefits apply when you receive services from an out-of-network provider; you still have coverage, but may be billed for the difference between the provider’s charge and the allowed amount.
**NEW for 2018 – The Options plan has many changes for 2018. The Options plan now includes two network tiers for medical providers, changes in some deductibles and co-insurances, and the addition of a virtual visit benefit. Maintenance medications in Tiers 1 and 2 will move to a mandatory 90-day supply at either retail or mail order following a maximum of two 30-day fills at retail.
