Quest Diagnostics (Quest) recently notified 34,000 individuals about a breach of their Protected Health Information (PHI) which Quest became aware of on November 28, 2016.
On November 26, 2016, an unauthorized third party accessed the MyQuest by Care360® internet application and obtained PHI which included name, date of birth, lab results, and, in some instances, telephone numbers. The affected information did not include Social Security numbers, credit card information, insurance or other financial information.
Affected individuals should have already received notification from Quest.
